Deltoid: Pain Pattern and Treatment
The deltoid caps the shoulder and lifts the arm in every direction. It is unusual among these muscles: its documented pattern stays local, felt in and around the muscle itself rather than referred to a distant site, concentrated at the front or back of the shoulder cap depending on the fibers involved.
Attachments and action
Origin: The lateral third of the clavicle, the acromion, and the spine of the scapula.
Insertion: The deltoid tuberosity on the lateral shaft of the humerus.
Action: Abduction of the arm, with the anterior fibers flexing and internally rotating and the posterior fibers extending and externally rotating the shoulder.
Where the pain is felt: In the shoulder cap itself, at the front or side, worse with movement and reaching. Because the pain sits where the muscle is, deltoid pain is often blamed for referral that actually arrives here from the rotator cuff muscles behind it.
Common causes: Direct impact, sports collisions, vaccinations and injections into the muscle, repetitive lifting at shoulder height.
Patients say: “It hurts right here on the shoulder when I move it.” Pain with reaching sideways or overhead, quiet at rest.
When to get evaluated: Shoulder pain after significant trauma, with visible deformity or inability to move the arm, gets imaging before needling.
How this muscle is examined and treated
Examination distinguishes deltoid‑local pain from patterns referred into the deltoid region by the rotator cuff: resisted testing by direction, then mapping of the cap itself. When the deltoid is the source, needling is direct and recovery is typically quick. Assessment and treatment happen in the same visit.
