Gluteus Minimus: Pain Pattern and Treatment
The gluteus minimus is the deepest of the three gluteal muscles. Its documented referral pattern travels farther than any other muscle in this region: down the leg to the ankle. It is the classic sciatica mimic.
Attachments and action
Origin: The outer surface of the ilium, between the anterior and inferior gluteal lines.
Insertion: The anterior surface of the greater trochanter of the femur.
Action: Abduction and internal rotation of the hip. Working deep to the gluteus medius, it stabilizes the pelvis during single-leg stance.
Where the pain is felt: Down the lateral or posterior thigh into the calf. The mapped pattern reaches the ankle without nerve involvement. This is the classic sciatica mimic. The lateral fibers refer down the outside of the leg; the posterior fibers refer down the back of it. Both patterns run the length of the limb, which is why this muscle is mistaken for a disc problem.
Common causes: Sudden overload (a misstep off a curb), long drives, sitting on a wallet, compensation for a painful hip.
Patients say: “Sciatica” that does not behave like nerve pain: no true numbness, worst on standing up from sitting, walking helps and then worsens it.
When to get evaluated: True numbness, foot weakness, or reflex changes indicate nerve involvement. The first-visit assessment separates the two.
How this muscle is examined and treated
Examination distinguishes this muscle’s referral from true nerve pain: pressure on the trigger point reproduces the leg symptoms when the source is muscular. The muscle lies deep to the gluteus medius; dry needling reaches it directly. Acupuncture calms the amplified signal along the pathway. Assessment and treatment happen in the same visit.
