Origin: The outer surface of the ilium, between the anterior and posterior gluteal lines.
Insertion: The lateral surface of the greater trochanter of the femur.

Where the pain is felt: The posterior iliac crest, the sacrum, and the outer buttock, extending into the upper posterior thigh. Read by patients as low back pain. The pattern hugs the belt line and the sacrum; lying on the affected side compresses the trigger points directly, which is why night pain on that side is part of the picture.
Common causes: Prolonged standing, walking on soft or uneven ground, rapid increases in running volume, standing with weight on one leg, a fall onto the hip. Runners who add mileage quickly and workers who stand on concrete floors load this muscle for hours without relief.
